Catholic Charities of Southern Nevada is now accepting resumes for the Medical Case Manager position.

Position Summary: The Medical Case Manager works primarily with low-income and homeless men, women and families to provide ongoing support and expertise regarding their medical care through comprehensive assessment, planning, implementation, and overall evaluation of individual client’s health needs. Responsible for conducting assessments for homeless and low-income clients to create a health plan which identifies supportive services needed to obtain medical care. Additionally responsible for the necessary follow up with local agencies to assure needed medical services are initiated and continue as planned. Case Manager is expected to arrange client appointments and transport up to seven (7) low-income and homeless clients at a time in an agency van. See full job description.
